    Capillary barriers during rainfall events in pyroclastic deposits of the Vesuvian area

    In the present paper, the capillary barrier formation at the interface between soil layers, which is characterized by textural discontinuities, has been analyzed. This mechanism has been investigated by means of a finite element model of a two-layer soil stratification. The two considered formations, belonging to the pyroclastic succession of the “Pomici di Base” Plinian eruption (22 ka, Santacroce et al., 2008) of the Somma–Vesuvius volcano, are affected by shallow instability phenomena likely caused by progressive saturation during the rainfall events. This mechanism could be compatible with the formation of capillary barriers at the interface between layers of different grain size distributions during infiltration. One-dimensional infiltration into the stratified soil was parametrically simulated considering rainfall events of increasing intensity and duration. The variations in the suction and degree of saturation over time allowed for the evaluation of stability variations in the layers, which were assumed as part of stratified unsaturated infinite slopes

    Cabozantinib After a Previous Immune Checkpoint Inhibitor in Metastatic Renal Cell Carcinoma: A Retrospective Multi-Institutional Analysis

    Background: Angiogenesis has been recognized as the most important factor for tumor invasion, proliferation, and progression in metastatic renal cell carcinoma (mRCC). However, few clinical data are available regarding the efficacy of cabozantinib following immunotherapy. Objective: To describe the outcome of cabozantinib in patients previously treated with immunotherapy. Patients and methods: Patients with mRCC who received cabozantinib immediately after nivolumab were included. The primary endpoint was to assess the outcome in terms of efficacy and activity. Results: Eighty-four mRCC patients met the criteria to be included in the final analysis. After a median follow-up of 9.4 months, median overall survival was 17.3 months. According to the IMDC criteria, the rates of patients alive at 12 months in the good, intermediate, and poor prognostic groups were 100%, 74%, and 33%, respectively (p < 0.001). The median progression-free survival (PFS) was 11.5 months (95% CI 8.3-14.7); no difference was found based on duration of previous first-line therapy or nivolumab PFS. The overall response rate was 52%, stable disease was found as the best response in 25.3% and progressive disease in 22.7% of patients. Among the 35 patients with progressive disease on nivolumab, 26 (74.3%) patients showed complete/partial response or stable disease with cabozantinib as best response after nivolumab. The major limitations of this study are the retrospective nature and the short follow-up. Conclusions: Cabozantinib was shown to be effective and active in patients previously receiving immune checkpoint inhibitors. Therefore, cabozantinib can be considered a valid therapeutic option for previously treated mRCC patients, irrespective of the type and duration of prior therapies

    Field inoculation of arbuscular mycorrhiza on maize (Zea mays L.) under low inputs: preliminary study on quantitative and qualitative aspects

    Arbuscular mycorrhizal symbiosis contributes to the sustainability of soil-plant system. A field experiment was conducted to examine the effect of arbuscular mycorrhiza (AM) on quantitative and qualitative performance in forage maize (Zea mays L.). Within the project Sviluppo di modelli zootecnici ai fini della sostenibilità (SOS-ZOOT) a trial was conducted at the experimental farm of the Agricultural Research Council in Bella (PZ), located in Basilicata region (Southern Italy) at 360 m asl, characterised by an annual rainfall of approximately 650 mm. For spring sowing, two plots of 2500 m2 were used, one sown with seeds inoculated with AM (M), 1.0 kg/ha, and the other one with non-inoculated seeds (NM). After 120 days after sowing, when plants showed 30% dry matter, five replicates of 1 m2 per plot were used to estimate dry matter yield (DMY), while half plot was dedicated to the assessment of grain production. For each replicate, three representative plants were considered; each plant was measured for height and was divided into leaves, stem and ear. For each plot, the following constituents were determined: crude protein, ash, ether extract, crude fibre (CF), fractions of fibre [neutral detergent (NDF), acid detergent fibre (ADF) and sulphuric acid lignin] and phosphorus (P). Throughout the period of plants’ growth, no herbicides, organic or inorganic fertilisation, and irrigation water were distributed. The preliminary results revealed a significant effect of AM inoculation on forage maize DMY, P content in the whole plant, into the leaves and on the quality of steam. The M thesis showed a significant increase in terms of DMY in comparison with the NM thesis: 21.2 vs 17.9 t/ha (P<0.05). The mycorrhized whole plants [0.22 vs 0.17% dry matter (DM), P<0.05] and leaves (0.14 vs 0.09% DM, P<0.05) showed an increased P content. The stems of M plants showed a content of CF, NDF, ADF and Ash significantly lower compared with NM plants. No significant differences were observed for the other parameters. The preliminary study revealed a significant effect of AM seed inoculation on DMY and quality of forage maize cultivated within a low input system

    Hyponatremia as a predictor of outcome and mortality: results from a second-level urban emergency department population

    Background Hyponatremia is the most common electrolyte disorder and it has been associated with increased mortality. Aims This study evaluated hyponatremia as a prognostic factor for severity and mortality. Methods We compared the prevalence of hyponatremia among patients who died during the year 2017 (from 1 January 2017 to 31 December 2017) with the prevalence of hyponatremia among subgroups of patients, i.e. outpatients, patients hospitalized for more than 2 days and patients admitted in the intensive care unit (ICU). We also described the mortality rate and the prevalence of comorbidities among hyponatremic patients, according to hyponatremia degree (slight, moderate, severe), basal characteristics, comorbidities and their outcome (discharged, hospitalized or died). Results In our population of a public hospital setting, hyponatremia was present at admission in 17% of deaths, and the comparison between hyponatremic and normonatremic patients in terms of mortality confirms the hypothesis that this disorder is in anyway strictly associated with vulnerability and with a poor prognosis. Conclusions We conclude that hyponatremia is a predictive marker for a bad clinical course, therefore patients with this electrolyte disorder should be carefully monitored

    The preservative restoration of the great work of Pericle Fazzini (1913-1987), entitled La Resurrezione in the Aula Paolo VI at the Vatican, has involved the workers of the Fonderia Del Giudice (Nola), and (for free) the authors of this paper, as researchers of the Faculty Architecture “Luigi Vanvitelli” and of the Benecon Research Center, for over seventy days.. The University contribution was orientated towards the scientific project of its conservation and restoration, as well as providing a repertoire of diagnostic tests that have supported the intervention protocol, appropriately designed, applied by the Foundry and confirmed by the Directorate of the Vatican Museums and Laboratories.. The essays describe the multidisciplinary methodology, analytical operations as well as techniques that have marked all the development and implementation phases of the preservative restoration, from concept to project, from shipyard to communication. At the same time, particular attention was given to the spectacular architectural space designed by Pier Luigi Nervi.. In line with the Tenth Forum topics, the work was inspired by the minimal intervention – remote analysis and non-invasive interventions – accurately planned, for the best expected result, that has returned the dazzling image to the statue as Pericle Fazzini has realized in a mixed alloy of bronze and brass. The work has also provided material and structural integrity of the articulated metallic mass

    Pediatric Celiac Disease Patients Show Alterations of Dendritic Cell Shape and Actin Rearrangement

    Celiac disease (CD) is a frequent intestinal inflammatory disease occurring in genetically susceptible individuals upon gluten ingestion. Recent studies point to a role in CD for genes involved in cell shape, adhesion and actin rearrangements, including a Rho family regulator, Rho GTPase-activating protein 31 (ARHGAP31). In this study, we investigated the morphology and actin cytoskeletons of peripheral monocyte-derived dendritic cells (DCs) from children with CD and controls when in contact with a physiological substrate, fibronectin. DCs were generated from peripheral blood monocytes of pediatric CD patients and controls. After adhesion on fibronectin, DCs showed a higher number of protrusions and a more elongated shape in CD patients compared with controls, as assessed by immunofluorescence actin staining, transmitted light staining and video time-lapse microscopy. These alterations did not depend on active intestinal inflammation associated with gluten consumption and were specific to CD, since they were not found in subjects affected by other intestinal inflammatory conditions. The elongated morphology was not a result of differences in DC activation or maturation status, and did not depend on the human leukocyte antigen (HLA)-DQ2 haplotype. Notably, we found that ARH-GAP31 mRNA levels were decreased while RhoA-GTP activity was increased in CD DCs, pointing to an impairment of the Rho pathway in CD cells. Accordingly, Rho inhibition was able to prevent the cytoskeleton rearrangements leading to the elongated morphology of celiac DCs upon adhesion on fibronectin, confirming the role of this pathway in the observed phenotype. In conclusion, adhesion on fibronectin discriminated CD from the controls’ DCs, revealing a gluten-independent CD-specific cellular phenotype related to DC shape and regulated by RhoA activity

    Noninvasive ambulatory blood pressure (BP) monitoring is a developing method in clinical practice. Its interpretation needs reference standards stratified by age and gender. This study addresses ambulatory BP monitoring in elderly people with the purpose of quantifying the discrete and periodic variability of BP pattern over a 24-h period. The ABPM was performed in 92 clinically healthy subjects (45 men and 47 women) ranging in age from 76 to 102 years. The results refer to the time-qualified mean values with their dispersion, to the circadian rhythm with its parameters, and to the daily baric impact (BI) with its variability. The conclusion is drawn that BP preserves its nychtohemeral variability and circadian rhythmicity despite old age. The daily BP mean level and BI in older people in good health are comparable with those of young subjects, suggesting that humans surviving into old age are characterized by a eugenic control of their pressure regimen
